RexB typed - When doing this, both the P3At and the LCP have responded with a click at least once. Somehow, the hammer wasn't cocked. How they get that way with a round racked into the chamber I don't know.

That is how it functions, DAO double actions only. Round goes in the chamber from racking the slide. Once the firearm is in battery the hammer on the DAO P3AT and LCP is at a full rest against the slide. The trigger fully sets the hammer before engaging the sear to release hammer striking the firing pin.
