Where can I order a kydex belt attachment for a small leather knife sheath?

It is a blind horse knife and sheath for neck wear and I would like to wear it on my belt. Maybe attached with chicago screws.

A Tek-Lok or these pre-formed loops come to mind. Kydex (also available there) is not that difficult to work; you could easily make something of your own design to fit your needs.
