Form for doctors to sign?

There was a thread quite some time ago that contained a link to two documents that a person could ask a doctor to sign if the doctor asked questions about guns/firearms storage/possession. The docs were in PDF format (I think). Does anyone have the link or remember the source?

While a nice idea to bring a doc up short when they present you with the "guns in the home" questions, the form is useless.

No doc is going to sign it, and you can't make them. All you can do is take your business elsewhere - that is, unless you're in a rural area with no other real options for health care.

Just refuse the questions - I have yet to hear of a doc who, in violation of the hippocratic oath, refused to treat a patient for declining to answer the "guns in the home" questions.

Just my $.02 - keep the change.
