I had BigJim at c5 Leather (Kansas City, KS)
http://www.c5leather.com/ build me an IWB holster for my full-size 45 Auto, and while he was at it, I had him make me a belt to match. Then I got him to make me a single magazine carrier, and a double magazine carrier.
The holster fits the gun just like a glove -- every little part on the gun is imprinted inside the leather of the holster, and the same is true for the magazine carriers! If my body could do it, I could literally run, jump, do somersaults, etc. and the gun and mags would all stay in place! However, as soon as I draw the gun from the holster or the mags from the carriers, the belt is heavy enough and the holster snugly-fit enough into my waste that the gun and mags come right out with very little force needed and no movement at all of the holster or mag carriers!
I have found that part of the 'trick' is to have the components located in just the right position. Orientation of the gun so it can be quickly drawn - yet out of sight, hidden by a jacket, sweater, or even an un-tucked shirt and in the right place so you can comfortably sit without having to keep playing with everything. The way I have my IWB holster BigJim made is just behind my right hip (around 4 o'clock position). I find that right there is the best place for accessability as well as comfort! I can wear both the single and double magazine carriers, however, I typically end up just wearing either the SINGLE or the DOUBLE - as putting both on is not really necessary and I do run out of "room" on my belt. Not only do the three components have to fit on the belt, in a comfortable position and easily accessible from any position I may be in, but they also need to be in locations that work well when the belt is fed through all the pant belt loops!
I do need to keep the belt on the "tight" side in order to keep the gun where I want it and to prevent my pants from falling down!
The leather BigJim uses is crafted in such a way so everything (the belt, the IWB Holster for the full-size 45, the single and the double magazine carriers) is about 1/4" thick! That's right, 1/4" thick in almost every location! I was and still am, really impressed with this guy's workmanship!
He is out to about 3-5 weeks lead time, however, the quality of his work is excellent. If you have any problems specific to your own body and wearing his components, he is more than happy and willing to take it back and make it work for you! I sent two things back and he made them work for me, as I have a bit of a spare tire which was causing problems with the way two items ended up fitting me!
In other words, BigJim STANDS BEHIND HIS WORK, unlike a lot of people today!
