Yes the FTF could be a high seated primer. Did you try to fire that round again? Did it fire the second time? If so it could be a high primer. The leading could be a slightly too low pressure and/or a too hard bullet for your particular firearm. A too hard lead bullet and a starting charge might not orbituate your bullet causing that leading. Lead bullets can be a bit finicky to get working well but when OK they are a good inexpensive way to shoot. BTW I had problems with one revolver and lead bullets when I started. Ended up having the too small throats of the cylinder reamed to stop leading in my Ruger 357.
