Well it could be a heck of a lot of reasons. If you are handloading with shotgun wads here is a point you should not ignore. "WAD SLIPPAGE". It is that tendancy of the wad to rotate in the bore and the the rotational movement NOT being transfered to the slug. Have I found out that the hard way ? You bet. This is ol' school for those who have been at it a while.
(Please bear with me ) It is a note for those who are not aware it does happen especially at higher velocities. Is there a solution . Sure ! Just get that slug to bite in (not thru ) and spin WITH the wad. How ? Well the simplest way I have found is to knurl the driving bands of the slugs. I happen to use Lyman and the accuracy improvement especially at longer ranges is dramatic.
This works so effectively that I can dust the wad with MOTOR MICA for slug release and still maintain bore rifling engagement.

Here is an example. Which would you like to shoot? CORBIN makes a tool for it. You have to go slow with the 12 ga. but the 20 ga. is a piece of cake. Dave has a winner with this one.

By the way here is the wad I use for launching a 20 ga. Lyman out of a 12 ga Platform. This one is home made. (It is a laminated duplex wad )
