Columbus, Ohio PD switched over, or at least made it availiable. They seemed pretty impressed in the article I read. I know one of my local PD's has switched over to it. There have been quite a few. To find them all you'll have to start checking back-issue newpapers.

Cincinnati PD issues the MP9 and Columbus PD issues the MP40. Several other agencies in Ohio are also issuing these pistols but the departmental size doesn't really have as much influence as the other two.

not too sure about issue guns, however I do know one cop who carries an M&P9 compact as his BUG

Iowa State Patrol, M&P40
New Mexico State Police, M&P357
Rocky Mount, NC PD, M&P40 (I think)
North Carolina Department of Corrections, Prisons and Probation/Parole, M&P40
New Hampshire State Police, M&P45
Patrick County, VA SO, M&P40
I'm sure there are others. Some of the agencies above might be in the transition process to the M&P, but the decision has been made.
