The article says that the kid that was disassebling the paintgun in question was a 15 year old friend of the birthday boy.
 
Purely speculation, but the kid may have mistaken the venting of CO2 through that emergency relief hole as being the typical shot you get when unscrewing a tank from the ASA and acccelerated his unscrewing rather than investigating.
Whoever was removing the tank likely had nothing to do with the faulty assembly. As stated above, even if the relief hole had worked the kid probably just unscrewed it faster. I've done this personally myself. My tank is quite the cheap peice of crap, and for some reason lets a lot of gas go when removing it. I try to get it past that kinda on, kinda off area as quickly as possible. You can bet next time i'll make sure its pointed in a safe direction.
 
I saw a full coke can take off like a rocket one time after being shaken vigorously and then partially opened/dropped. It went about 100 feet horizontally, hit a wall, climbed about 20 feet and then crossed quite a ways on the ceiling before losing power.
