Need help with Form 4

I filled out my form 4 and sent it to the address on the form:

National Firearms Act Branch
Bureau of Alcohol, Tobacco, Firearms, and Explosives
PO Box 73201
Chicago, IL 60673-7201

It was returned unopened today, marked "Return to Sender- Refused by JP Morgan Chase Bank, NA"

Do I have the wrong address?

I am using ATF Form 4, Revised November 2004

I think the current address is somewhere in Atlanta, let me see if I can find it.

Don't just resent the current form.

If the address is wrong on the form you used, you may have an obselete form.

Check with the ATF to make sure not only of the address but also to see if there is an updated form. They may reject your Form 4 out of hand if you use an obsolete form.

You should find out for sure before resending it as otherwise you might run into a new problem that will delay the whole thing that much longer.
