Inside. There is a lip that holds it in place as you draw

I have been very satisfied with a Fobus paddle ( I carry my Glock 21 daily ).
I have been less impressed with a Houston paddle I bought for a smaller pistol. The paddle "lip" does not always keep the holster in place when I draw.

Wear on the inside

I have a bianchi paddle holster for my glock 19. I love it. You wear the paddle on the inside of the pant.
 
I too prefer Fobus. The paddle insert that goes into the
waistband has a nice rubberized coating that sits against
the hip. Definitely makes carying much more comfortable.

A word of warning: I bought a Fobus for my S&W 642 revolver,
and the draw was so tight and awkward that I had to return it.
For semi-autos, this holster is just great.
 
Well I popped for an Uncle Mikes for my G19. I wanted a paddle for ease of removal. This thing grabs my pants/belt like an alligator, so...so much for easy removal when I cross the line into Illinois. The plus side is it appears the paddle adds to stability.
 
I have a DeSantis Viper paddle holster for my USPc that I like alot. The draw is smooth and the paddle is designed so that the holster stays where you put it.
I've tried Fobus and Uncle Mike's and have had trouble with both - the Uncle Mike's didn't want to let go of the gun would sometimes pull out of my pants and stay attached to the gun when I tried to draw. I had a Fobus paddle and holster pulled apart from each other during a physical altercation so I ended up with the paddle still in my pants and my gun on the ground inside the plastic holster.
I suggest you shop around and buy quality, when it comes to holsters you definitely get what you pay for.
 
I have a Hellweg Paddle Speed holster I used for competition and occasional everyday carry. It's tension adjustable and and makes for a fast draw.
My G23 carries well in this holster.

Paddle in, gun out. I currently use a Fobus Paddle for a full size 1911. Carried it yesterday from 4:30 a.m. to 9:30 p.m. with no issues. Very comfortable. I was in and out of a truck, and on and off of an airboat all day. Again, very comfortable. And it is great for easy on and off for those quick trips to the convenience store at 10:00 p.m. after stripping down to in-house attire.
 
If you pick the right one for you handgun, they can be very very nice! I have a few now for my Glock, a HK and my 1911 and am glad I've discovered them! :D
