Possible Problem On Star BM

I ran my Star BM through some more rapid fire drills today, and after about 200 rounds the bushing ejected itself! I'm not too familiar with these single action semis, but as far as I can tell the thing is only really held on by friction fit. Is this going to keep happening and do I need to put some lock tite or glue in there to hold the outer part of the bushing to the slide?
 
The bushing is actually held in place by the forward pressure from the guide rod (which keeps the bushing from rotating) and the bushing's tab on the outer diameter which fits inside the channel at the front of the slide -- essentially the same as on a 1911.

Guide rod not seated correctly, perhaps? Maybe a weak recoil spring? I've seen this on one pistol -- the tab that fits inside the slide had peened itself through use such that it was extremely loose fitting.
 
The tab that locks it to the slide is indeed pretty loose, but it seems intact. Probably from years of wear. I'll lock tite it to give it a bit more grip.
