Problem with Lee Factory Crimp Die

I'm loading 45 ACP West Coast Plated 230gr. RN. I have cleaned and lubricated the die. The tapered side of the crimper insert is down. The die is functioning as a bullet puller. I don't seem to be able to change this. Have played with mouth bell to no avail. Anyone know what is going on and how to get back to reloading?
 
Let me make sure I understand the situation.

The Bullet is sticking in the Lee Factory Crimp die..??..

If so.. I've seen that a few times also, usually with the hated A-Merc brass. I've also seen it with some very thin Remington brass.

With the Remington brass I just cranked the die down all the way and it work. With the A-Merc, I just tossed the brass.

Also, I guess it's always possible that the bullet itself got squished into a wider diameter somewere in the process..
