As the title states, a few simple questions about form $ stuff.

1: What kind of check, personal, money order, cashiers????

2: On the fingerprint card, it states under "signature of official taking fingerprints" "employer and address" Is this my employer and address, or the employer and address of who took the prints?

3: What kind of envelope, I got one of those cardboard "mailers" is that g2g?

1. Your choice. They aren't worried about if your check will bounce, they know where to find you.
2. I don't think that is necessary Link here
3. Yes. A regular manilla folder is fine too. You could probably fold it up and cram it in a regular envelope if you wanted to but I wouldn't risk it coming back and wasting more time.

1. I have always used a personal check and never had a problem.

2. Employer and Address of person taking your prints. I have an instruction sheet from ATF for the fingerprint card and that is what is on those instructions so that is what I have done.

3. I have always used a large manilla envelope. When ATF sends the approved Form 4 to the seller, they just fold it and put it in a regular business size envelope.
