On a single or SxS shotgun, does 18" refer to the entire length of tubing known as the barrel, or is it 18" plus whatever the length of the chamber is?
 
Length of barrel measured from the muzzle to the breach face.
Yes, it includes the chamber.

All firearms barrels are measured that way except revolvers.
They are measured from the muzzle to the front of the cylinder.

It is the same for ANY type of shotgun - close the breech or action, place a dowel down the barrel, mark it and measure - voila - you have the barrel length
