Going to be fast:
Might have one oversized chamber. S&@ has done this before.
Have it checked.
Limp wristing? With guns that recoil at a high rate of speed, if you allow the gun to move too much, the bullet, usually the last one, can creep in the cylinder, causing the cylinder to lock up.

Do the grips fit your hands? If the grips are too small, on such a gun, it's hard to shoot with heavy loads, and, it can move too much.

The cleaning of the chamber is possibly a problem. Has this happened before?

Tim Sundles, Buffalobore is usually a great guy, straight and honest. If S&@
did change their specs, then the rounds could be creating pressure over spec,
and the rounds might stick, and not eject, this usually only happens if the chamber is oversized.

I'd mark the chamber that sticks, and see if it happens with one chamber consistently.
