Saw an A/D this weekend

Was pulling trap for a tournament Saturday. Shooter on station 1 loads 2 shells into the magazine of a Benelli, opens chamber, releases.....BOOM. Fortunately, the muzzle was pointed at the ground, and the impact was 3 feet away.

As far as we can tell, the firing pin never reset, so when the bolt slid forward, it struck the primer. Anyone seen this happen? Anyone have another explanation? Finger off trigger, primer was struck, not sure if chamber fully closed--when I examined it, the bolt was half-way open, the next round was trying to feed, so it was jammed up.

Decades ago, had a cheap Sears Model 99 Savage E, in .243 caliber scare my wife.

I was in the bedroom and chambering some reloads I had recently made up. But remembering rule No. 2, I had the gun pointed in safe direction. The cheap M-99 had a sear malfunction, when the cartridge was levered in...it went boom in the house.

Wife screamed my name, I hollered back...everything is OK, except this cheap piece of crap that just malfunctioned.

Had a new sear put in rifle and promptly sold it.
