I was plagued by dropped magazines pretty badly in my Shield 45, so I was on the lookout for remedies. Apparently S&W issued new magazine followers that resolve the issue, and a thread at their forums explained the problem and solution:

I modified my followers to resemble the factory followers, and the dropped magazine issue had cleared. See the picture. Fortunately, the magazine body did not need any changes.
This success comes as a big relief, because I tried other common guns in this format and found them lacking. Finally can get rid of the Kahr P45 that I carried before. The Shield is fully reliable aside from the magazine drop problem.
