Thanks for the sub. Nope... no explanation at all. I assume I had a failure to fire, then FTE, and then chambered a round on top of it. It cleared in the usual way, but you can see I also had a failure to fire directly afterward. Possibly ammo problem, or crud under the extractor. I suppose it's also possible the first cartridge didn't seat fully which prevented the firing pin from fully contacting, but in that case I'd hope the slide would be out of battery and the hammer wouldn't fall at all. Ammo was MFS 115 grain FMJ; I had a couple of duds throughout the day and I don't think I'll buy it again.

I keep my guns pretty clean but I've never torn apart the slide on the SIG so maybe there's some crud in the firing pin channel and extractor area. Nothing horrible is visible and the extractor looks in good shape.

It's the German-made one with the stamped slide and pinned-in slide block. The only issue EVER I've had has been the slide not locking back on some mags with some (weak Remington/UMC) ammo. I've never had a feed malfunction until that one. The mags were Mec-Gar 18-rd ones and they're usually great. I'm going to disassemble/clean them also.
 
I just checked and on the SIG if you hold the slide rearward a VERY small amount the hammer will still fall, but the shelf below the firing pin prevents it from being hit by the hammer. So either a dirty chamber preventing full chambering or out of spec ammo is IMO the most likely cause, considering I've had zero feeding/extraction issues in at least a couple thousand rounds since new.
