Stumped By Misfire

A friend had a live round fail to fire, fail to eject, and a 2nd round feed & jam when he tried to cycle the bolt. The first round had to be cleared with a rod from the muzzle end. No apparent damage to it or following round, and both later functioned normally. Ar 9 mm pistol which had over 200 rounds of the same ammo, all from the same 1000 round case he bought. No other incidents. Any Ideas? Thanks
 
In general. FTFire can be caused by many different things. The most common is round did not fully chamber and disconnect had it locked out. firing pin did not hit primer. A dirty chamber can cause this too.
