Define concealed. An IWB holster with the top of the gun exposed, wearing a cover garment, the VM-2 is as good as it gets. Wearing an IWB with the shirt tucked over the gun, completely concealing it, with no cover garment, i'd say Will Bill's Undercover.
The best concealment holser is one that works. IE: is comfortable to wear, conceals the weapon, holds the weapon securely and safely, and does no damage to the weapon. Try some different types and find out which is the best holster for YOU. Personally, I like my "pager pal" holster, but it's not for everyone.
has always worked for me. I carry a 5" 1911 under a polo shirt a lot and they've been comfortable and, so far, not noticed. El Paso Saddlery, Milt Sparks, BladeTech, Alessi, and Bianchi all come to mind, look at all of them and decide what you're comfortable with. What works for me might be terrible for you.
Had it for three month now and it is a very good holster for IWB. Keep the gun (H&K USP .40S&W compact) in the same position even when working on the yard (just a test I did). It is also extremely conftable to wear all day even in 80 degree weather in Florida. It is also fast to draw and re holster. The only thing is that it rides higher than the Mexican style I used to wear and now I can't conceal it with only a T-shirt. Only with dark overzise t-shirt is possible. All my untug shirts work well with it. Althoug is expensive I think you get good value for the money.

Kramer Leather makes an outstanding IWB. I have an IWB#3 (extreme FBI cant) for my P220 and just got one for my new P239. Even with my spare tire and the full size P220, I could wear it from morning to bedtime comfortably. I also have their 1.5" double thick horsehide belt and it's great. All standard hosters are hand-molded horsehide and ones for common handguns ship in about 2 weeks.
For a large gun like a USP45 you need a dual clip (or loop) type holster like the Milt Sparks VersaMax II, Watch 6 or an HBE COM (with a gun as large as the HK I'd say go with the COM, not the COM2/3).

Sparks VM-2 and the ubiquitous Summer Special are my favorite deep concealment IWBs, but the Brommeland Max-Con V and VI will give the Sparks stuff a good run for it's money. Look at Alessi's new PCH as well. It's super comfortable to wear, yet it pulls the gun in real tight against your torso.
