I'm sure back when the revolver was nearly the universal LE handgun, there were instances when reloads were required. In fact, I know this happened in the FBI Miami shooting.
And it certainly happened in the CHP Newhall affair. Four CHP officers killed by 2 very bad guys.
But back to the point, the only two instances I've heard of American citizens or LEOs reloading revolvers in a self defense shoot out have already been mentioned.
There have been many cases of Law Enforcement having to reload their revolver. Ever hear of a New York Reload? Ever read the stories of some of those guys?
New York reload is the fastest!
That would depend on where that second revolver is carried. If it's in an ankle holster or even a pants pocket someone that has practice can reload a revolver already in the hand faster with a speed-loader or moonclips off the duty belt than reach for a deeply concealed bug.
