    This usually (but not always) happens when someone dry fires the gun with the sideplate off. It is fixable on a steel frame, don't know about an alloy frame, but S&W will probably just replace the gun anyways

    There is a screw at the bottom of the front strap (bottom of the grip frame, trigger side). Make sure it is all the way in and tight. It is most likely your problem. Edit: the horizontal screw in this pic:

    Lighter bullets are a big part of the problem, also the nightguard sights are regulated for a "combat hold" where the front sight covers the area you want to hit instead of lining them up right below it. Where you firing single or double action?
