When I called they told me they wanted 90 days before anybody calls but she checked for me then two days later I had mine. I had to keep trying to call I left messages but never heard back I tried about 2 or 3 times a day for almost a week before someone finnaly answered.

First one under 2 weeks (10 or 11 days, IIRC).

Second one about 5 months.

Can be mailed or faxed, either one. Most will accept either, some just want them mailed.

Took almost 90 days for mine. Relax, it'll get there.
