Here is how you do it. First you need to live in AL, AR, AK, AZ, CO, CT, FL, GA, ID, IN, KS, KY, LA, ME, MD, MS, MT, NE, ND, NV, NH, NM, NC, OH, OK, OR, PA, SC, SD, TN, TX, UT, VA, WA, WV, WI, or WY. Then obtain the ATF form 4 from the ATF here;

http://www.titleii.com/Forms.htm . You will also need form 5330.20 (to attest to your US citizenship), and fingerprint cards also available from the ATF.
When filling out the ATF form 4, make sure you do not leave any portion blank unless it says to. Block 2a is your personal info. Block 4a the manufacturer’s name and address. 4b, is silencer. 4c, is caliber. 4d, is the model name. Write N/A in block 4e, length in 4f. 4g, is the serial number. 4h, has the silencer identifying data. Blocks 5 through 8 are for dealers. Hopefully you answer no to all in blocks 13 and 14. Block 15 is where you write in why you want the silencer, something like enhance firearm collection or all legal purposes will work.
Attach with tape or glue a passport sized photo of yourself in block 16. Printing out pictures on your printer is suitable. It should be recent. Sheriff signs below in the block 17.
If you use a trust or Corp. then the photos, finger prints, sheriff signature and 5330.20 are not required. You need to include a copy of the trust and also the $200 tax. It is best to use a personal check, this way if it is lost you can cancel it and you will know when it is cashed. If you go through a dealer (only required if going across state lines) then the dealer will make sure the forms are filled out correctly.
