General Bill of Sale for a Firearm

Does anyone have a good general bill of sale for a private party firearm transaction?

I don't generally make one, but if you are going to you may want ot include DL#, serial# of gun and date.

Here's an example of the text I use:

Bill of Sale of Personal Property – April 4, 2009

In consideration of the sum of four hundred and eighty dollars ($480), the receipt of which consideration is acknowledged,
the seller, Your Name, Your city and state, SELLS and DELIVERS to the buyer:

Buyer Name
Address
Driver license #

the following personal property:

Sig Sauer CPO 220 Carry .45 pistol: Serial number XYZ
One 8-round Sig .45 magazine

One short reach Sig trigger for the Sig 220
