Sorry for pushing my opinion but, with arthritic hands (and they will worsen) I'd go with a semi-auto carbine... either 9mm or .45 ACP. He'll not likely have any trouble handling one of those because it's easier to hold onto and shoot. Okay, I'll shut up now.
 
No need to apologize to anyone for that opinion, Mike, it's a good nomination, any pistol caliber semi carbine. Might depend on his fingers and the loading sequence, etc., though, real hard to call that. Somewhat like a tip up barrel 380 pistol, in that regard.

Me, a bit of arthritis in one hip and one foot (good thing moving whilst shooting is purely optional), but I have seen some old fingers that make me glad it ain't my hands, you know.
 
I was going to suggest a Model 10 or something like that...but the small carbine might be perfect. The problem is carrying it around. I usually have a handgun near because they are easy to tote from on place to the next.

Maybe a MP5 sized weapon with an oversized charging handle so he can grab it w his whole hand would be a nice compromise.
 
Here's a stupid question, if he ended up with say a 4" Smith, would one of the hammer extensions that come with Marlin lever rifles fit on the hammer? Seems like it would make things easier for someone with arthritis... It seems like it would make cocking with the palm of the off hand a possibility... Just a random thought I had...

and they go so well with casual wear fashion as well, especially so with a fine fashion sense about what you choose to carry :)

I like your thoughts on over sized cocker for a semi-carbine, though, and some preloaded clips.. er, mags

yeah, hadn't thought of that, 45G, hammer side extension, ought be easy enough to fix one up

Just seems easier than trying to wrap an arthritic thumb around one of those tiny spurs. Hell, it's uncomfortable trying to do it with my Police Positive without arthritis. Over sized grips would be nice as well, the tiny stock K-frame panels wouldn't be very comfortable.
