Help Needed - Starbucks Petition Technology

I want to create pro-RKBA petition with the same functionality as the Brady Campaign/Starbucks petition found here: http://www.bradycampaign.org/starbucks/ (or sometimes it's on the main page)

I have XHTML/CSS and graphic design skills but lack the programming skills. Anyone know of a simple turn-key solution or have the knowledge to help out?

Any input is appreciated. I know there are free, open-source solutions available but I wouldn't even know where to begin.

My end goal is to have something like the Brady Campaign/Starbucks petition except to support RKBA and the open-carry movement.

From personal experience, I can tell you that SurveyMonkey.com is unreliable. My school uses it to conduct student surveys, and we've had a huge problem with surveys disappearing, or being unaccessible by some students while others have no issues.
