I'm Late, Does Anyone Have the NRA Survey with option to join lifetime for $300?

I came in one week late for the $300 lifetime membership. I kept calling and trying to get someone to work with me. One lady explained that: If someone joined with a lifetime membership in the last 5 months, they should have received the paper survey in the mail, which should have printed upon it the option to join lifetime for $300 (idea being get someone else to sign up for that).

With that, does anyone have this paperwork? I would really like to get in on the $300 lifetime membership, and this is the only way it is going to happen.

I just checked the www.nra2013.org website and it looks like you can still join using another life member's info to gift the $300 life membership to yourself. Look at the other $300 life member thread, it has all the info on how to do it.
