If you don't mind a bit of extra weight, the PPK or PPKS are the Rolls Royce of .380's

Possibly you could add the SIG P230 and P232 to the Rolls Royce registry as well?
 
My KelTec 380 is around 10 yrs old also. Never had the 1st problem with it Keep loaded with Corbon DPX . Got several P-32 in house also .
 
For actual pocket carry, a P3AT or LCP (very nearly the same pistol) are the way to go. Like others have said, I also use the DeSantis pocket holster.

I have other 380's, but I can't effectively conceal them in a regular pants pocket with no shirt hanging down to cover them.
 
I'd say save for a bit more and get the Sig P-938 - unbelievable that you can get a 9mm in a .380 pocket pistol frame. Yeah there was some trouble with the slide-stop spring (easily replaced if it even proves to still be a problem). The smallest all metal 9mm pocket pistol on the market. The ONLY difference in size between the P238 (380) and P938 (9mm) is the smaller grip, obviously due to accomodating the smaller round. Just bought Mrs Tiger a P238 and after her FIRST trip to the range she fell in love with it!

Admittedly - since I carry cocked & locked - a thumb safety and frame size were paramount for me. That's why the Sig ranked number one on my list. Not everyone likes or wants the thumb safety.

If you do...and don't want to bust the budget...have you considered the Smith & Wesson M&P Shield?

...having been one who couldn't wait, and having JUST done this same analysis - I almost bought a S&W M&P Shield in 9mm. I think it may be even slimmer than the P938, while the overall height of the standard magazine (not the extended magazine) made it a competitor with the P938 as well as the P238 - but the price of the Shield ($360 to $400) kept forcing me to consider it.

If you DON'T want the external/thumb safety...

The Kahr CW380 was an absolute joy to hold and shoot. The only drawback for me was the lack of an external safety. Ergonomically, it was THE best (for me).
