Is a NICS form a "Firearms Application" in New Jersey or anywhere else?

To apply for a Firearms Purchasers Identification Card or a Permit to Purchase a Handgun in New Jersey you must answer this form question:

Have you ever had a firearms purchaser identification card, permit to purchase a handgun, permit to carry a handgun or any other firearms license or application refused or revoked in New Jersey or any other state? If yes, explain.

Does anyone know if a NICS form is a "Firearms Application" in New Jersey or anywhere else?

P.S., I know that if you apply for a permit to carry in New Jersey you will be denied and that is something you must answer on the New Jersey form.

P.P.S. Fortunately I have not had this happen, but there are a lot of innocent reasons for having a NICS check refused.

The FBI NICS does not have forms.
The information they ask is found on the ATF Form 4473.
It is a record of a firearm transaction and not the same as a "firearm application".
