Is it normal for scope rings to dent your tube?

For my first precision rifle set-up, I bought a Savage 10FP, a Bushnell Scopechief 4-14x50mm AO scope, Weaver 46 2 piece mounts, and Burris Signature Zee rings (with the polymer inserts). I loc-tited the whole shebang down and had great results, until...

My front bottom ring screw became loose. I went to tighten it, but it was stripped and turned freely without tightening. So I took the front rings off to return them to Burris, and I discover that my scope is damaged! The scope tube is dented from the polymer insert....instead of a flat squeezing fit of normal rings, the rear side of the front ring's inserts left a very noticeable indentation and bulge (kind of like if you look at the side of your index finger while curling it, you see the creases that correspond with the opposite sides of your knuckles and the bulge of the flesh in between). The front rings were mounted just before the angled bell of the objective. The inserts also slightly scratched the finish of my scope tube. Here's my questions:

1) Would the bulge have possibly damaged the scope other than cosmetically?

2) Does this kind of thing happen normally?

3) Obviously Burris needs to replace my front rings, but should they also pay for a replacement scope? The rings obviously didn't live up to their reputation or Burris' claims of leaving no ring marks.

If the tube is dented it's damaged. What did you use to tighten those rings so much? Nothing more than a screw driver is required.
