Mossberg trouble

I have a Mossberg Model 88 12 ga. Latley after I have fired about 20 rds. It doesnt want to eject the spent shell, and a live on falls out of the bottom. What is going on?
 
Something is messed up with the shell holder/follower. Take it apart and put it back together (find directions online) and see if it fixes it. Sounds like something didn't get put together right, or something broke. Ive got a 500 but have yet found need to take it apart so I cant help more than that.
 
I tried taking it apart, giving a good cleaning and inspection, but I cant find anything moving parts broken. But there is a chip about 1/8" in diameter inside the gun, on the top, about 1" behind chamber were the live rounds go in. It has been there for about a year now. I have had no problems till recently. I dont know the exact words for all the parts on the gun, I am still learning, so sorry I cant be more specific.
 
Original stock and fore-end? Sidesaddle? My original fore-end wouldn't clear the sidesaddle and about half the shells would just drop to the floor. It FELT like it was, but once I took the side saddle off, it worked fine again.

Keep in mind that without seeing the gun we can only guess .

First problem of not ejecting needs to be clarified a little - where does the empty casing end up ? Does it extract it out of the chamber and then drop it into the action rather than ejecting it out the port ?

The second problem of dumping a live round on the ground is an indication of a need to adjust a shell stop. The live round is pushing past the stop rather than being controled by it. Make sure your chamber is clean, and also take out the shell stops and clean under them as well as cleaning the stops themselves. If your problem is still there, watch the operation of the shells feeding from the magazine to get an idea where your hicup is occuring.
