After a thorough cleaning and lube I took my new TCP to the range and fired off 100 rounds.
I'm really happy with the way it shoots for such a small gun, its been about a year or so since I shot a Ruger LCP, but if I'm remembering correctly this gun shoots noticeably better.
Anyway, I had 2 failures to fully chamber the round (around the 75 round mark) a little tap on the back of the slide made it chamber.
Is this a problem or simply normal break in procedure? I couldn't really see the round binding up on anything.

Maybe I'm being paranoid but I'd like a gun I'm going to potentially trust my life to to be 100%

It may be that the recoil spring is just not broken it yet, or it may be too light. My ruger lcp would fail to return to battery every now and then before I replaced the silly 9 pound spring with an 11 pound spring. Now it runs like a champ.

The TCP is definitely going to be my next .380 purchase, the ergos are great on that gun. My LCP isn't bad either, especially since I added the pachmyer grip sleeve.

Try some copious slide racking and see if that helps break it in, I suggest wearing work gloves.

I polished the chamber, breach face, and outside of the barrel a bit, it feels a lot smoother now.

Despite what many say about guns and break-in periods IMHO all mechanical assemblies need some period of break-in. In fact on all new guns, I will disassemble them and LIGHTLY run a craytex or hard Arkansas stone over corners and sliding surfaces to reduce roughness or burrs. Then lightly lube.

With the exception of the Highest Priced hand guns which have received extensive hand work, mass produced weapons have some roughness as a result of the manufacturing process when they are shipped. During the firing process the weapons slides these surfaces together and they mutually smooth each other. In a comical way like breaking in new shoes. It takes some time until they are really comfortable :)
