Not exactly “reloading” but some people can have problems with equipment that is relatively simple….

Wow! How could you even do that? Talk about unlucky and lucky!
I think I figured out how......borrowed a short arrow from somebody....and thought he could guide it in mid air????
Speaking of unlucky and lucky.....and a little stupid too.... I did that to my index finger with a 16 penny nail gun while framing a house when I was young......held the stud too close to the end. At least it didn't go through the bone (the lucky part, like your archer)....healed nicely.....only a tiny white spot to show for it. ;)

This ones not a reloading issue rather a deliberate, even if, in the end, unintended, bore obstruction.

I’ll give the fellow the benefit of doubt that he didn’t leave it in there because he thought it would be OK.

yeah, I saw those two pictures too...and a third one that showed the bloodied shooter being stretchered into an ambulance. Thought I'd never see it again, but I did. At least this one didn't get hurt so bad.

My wife's nephew (who lives in a little Colorado ranching community) sent a Remington 700 chambered in 7mm Magnum to me, and asked me to take it to a local gunsmith to see if he could rebarrel it. It looked exactly like the picture you posted....banana peeled barrel....and caused by the exact same bore obstruction....a laser bore sight. He said his son was sighting in a new 4-12 power scope, and forgot the bore sight was still inserted. It was factory ammo, as he doesn't reload.

The Gunsmith took pictures of it (I didn't, but wish I did). Then he told me, "No way in hell, would he rebarrel it." Said the receiver most probably had hairline cracks not visible without a microscope, and he didn't want anything to do with it. ;) So I sent it back to him, and he said he was going to display it on the mantel. I think he took it to a land fill.....what a waste.

I also saw the episode of "MythBusters" based on the same pictures you just posted, where they said that couldn't happen with just a laser bore sight.....maybe maybe not.

Most probably that depends on the caliber. At least one high pressure magnum certainly did. Don't remember what caliber the one in your picture was...do you know?

IMHO everyday when I check out some of the few sites I am on, there are numerous posts that are cringeworthy. "close enough" "just send it" "as long as it goes bang" etc... is the "advice" that really makes me cringe.

There are many many variables that effect ballistics and not paying attention to details is the recipe for disaster.

There is a daily barrage of lackadaisical "will this be OK?" questions posted, seemingly not to hear where they went wrong and fix it but instead to justify what they have already done so they don't have to.

Just recently I took apart about 80 M80 ball rounds I put together because I did not leave myself sufficient documentation about what they were, and with all the summer projects going on I just couldn't remember. (there's a lesson there.)

I'm far from perfect and I recognize that, so I'd rather back up and start over, rather than back up and drive to the hospital...
 
This happened early in my reloading career.
Didn't know the difference between a squib and hang fire, I didn't even know such things existed.
My 9mm made some sounds that hadn't happened before. One was just a tad different. Went to remove the slide and BLAAM!!!
Luckily my hand has pretty much fully recovered and I am now seriously OCD about my reloading process.
No squibs, hang fires or odd sounds coming from my reloads.:)

By the way, it is true, burning human flesh does indeed smell like bacon!
Mine also smelled like gun powder. Haven't tried sprinkling any on my breakfast bacon though.
I also liked the smell of a burnout in my 72 Chevelle!!!
 
Well, that's exactly what it smelled like, an early Sunday morning breakfast with bacon and a dash of gun powder for seasoning.
Only thing missing was the smell of fresh brewed ☕.
 
Should check out Kentucky Ballistics on YT.
He had a .50 something blow up on him and it damn near took his head off and gave him a ready to go open heart field surgery.
He survived but barely!
