I posted this on a local forum where I live but I figured I would post it here and see if anyone has seen this issue before. The gun is back at S&W but I have not heard anything yet.

I took my new M&P 340 to the range and the of the first 15 rounds I fired (all .357 mag), not one hit my 8.5x11 target 30 feet away. I returned a few days later with some real mild .38sp and one of my other J-frames to compare it to. I also brought a poster board so I could see how bad I was missing by. the first group of .38's out of the .38 j-frame were about a 2.5 inch group dead center. I loaded 5 rounds of .38 into my M&P 340 and they were ALL OVER the place. Mostly low and right by about 12-14 inches. When I went to load 5 more rounds in to try and figure it out I notice a lot of gouging on the frame on the left side in line with the edges of the cylinder. I had no idea where that came from. I fired 5 more with the same result. This time when I opened the cylinder it fell right out of the gun and on to the floor in one piece, the crane and all. As I was leaving I notice some fairly good sized bullet shavings on my bench. Maybe the cylinder is misaligned or the barrel is off and the bullet is shaving itself as it makes the jump into the forcing cone?

Has anyone ever seen this?

Hopefully they repair it cosmetically as well as mechanically. It would be a bummer to have a brand new gun look like this:

The damage on the shaft (far left circle) is actually a piece of the shaft broken off where the bolt goes through the frame to hold it in.
