Remington 1100 cycling issues

I have a Remington 1100 2 3/4 12 bore that I took to the range today. The problem is it would not cycle the last shell. I had to manually pull it out of the magazine a bit to get it to chamber. I tried several different loads and it happened with all of them.

When I took this gun apart for the first time to remove the magazine plug i tipped the gun down a bit and the follower came out along with a large flat metal washer except no hole in the middle like a normal washer. Im not sure what order these were in the gun or if the "washer" is supposed to even be in the mag. Doesn't show up in the exploded diagram in my manual. Any thoughts? If the washer is not supposed to be in there then maybe thats where the cycling/feed problems are coming from.:confused:
 
There is not supposed to be any washer in there, that I do know. Do you have a plastic follower? A lot of times these can get chewed up and get imbedded grit and cause issues. Try cleaning the mag tube well with a 10 gauge brush and make sure the follower - in an empty tube - will fall all the way down to the receiver with no drag before you put the spring in.
