A very weird problem... I have a Remington 582 that I have owned since 1972. It is a very accurate gun but will only fire when I feed it by hand. I.e. when bullets are loaded via the feed tube they will chamber correctly but the firing pin strike does not cause the bullet to fire. This happens 100% of the time.

Any ideas what is causing this?

Clean your chamber with a brush wrapped with steel wool and oil. Check the mouth of the chamber for a ding from the firing pin, it can be ironed out if present.
My opinion is that when you feed by hand you push a round all the way down in the chamber, where as when it is loaded automatically, it is not completely chambering, and the blow from the fp is being used to finish chambering the round.
