On later models the bolt face doesn't have the shell retainer which holds the casing in place until it hits the ejector.
The gun fires, the bolt recoils, and when the casing clears the barrel it can fall into the ejection port. The bolt continues backwards, picks up a new round, which feeds half way into the barrel. Now you have the old casing blocking the bolt, 'The dreaded double feed'.
