My 12 ga. has a "Modified" choke, and I'm practicing and getting better at trap/skeet. Will changing to a "Skeet" choke really make any significant difference?

It will at Skeet.
It's a close range fast moving game.

Maybe not so much at Trap.
It's a longer range game.

Both games are shot at very different distances if you are doing the real deal and not just throwing blue-rock in the air.

skeet choke is .005, modified is .020
Skeet's longest shot is about 21 yards or so
Trap's can be 40+

That being said, if you are on the bird, it doesn't matter at all if you want to use a Full on skeet

The modified choke is ok for 16 yard trap, an improved modified is even better. For skeet, get a skeet choke. In a 12 ga, use #8 shot for trap, #9 for skeet. An improved cylinder choke with #8 shot works for hunters clays for me. Using different amounts of shot from 1 oz/close to 1 1/4 oz/long shots.

Yes switching from Modified to a Skeet choke may dramatically increase the size of your pattern. With lots of small pellets at close range it should be a big help at Skeet (ideally w/ #9 shot, but 8.5, or even 8 would be ok)

Trap is longer range, and it sounds like tighter chokes and heavier shot (7 1/2) are the norm.

A good way to practise your skeet shooting is with a tight choke and long barrel, then use a skeet choke and longer barrel when you want to do it seriously.
I use a 28" barrelled gun with modified choke and 28gm 7.5 shot to practise, and a 24" barrelled gun with skeet chokes and 28gm 9 shot when I want to take it seriously.

Best guess, going to a Skeet (5 POC) from a Mod (20 POC) will give roughly 4" more usable spread at 21 yards, the longest distance of a shot at Skeet.

That's not inconsiderable. I'd also think about going to Cylinder instead, you may pick up another inch or so over the Skeet pattern.

Many choke makers consider Skeet to be 5 POC, but not all. Remington has a Skeet tube LARGER than bore diameter by 2 POC, more open than Cylinder.

Choke tubes are not that expensive, so a little research won't break the bank. Try a few things out and see what's working for you.....
