I am looking at picking up a Ruger SP-101 .357 magnum with a 3 1/16" barrel with fixed sights. My question is...can this gun have it's "sights adjusted" by a gunsmith if the sights are "fixed"?

Not really!!

Only option IMO would be an expensive conversion to adjustable sights. It could of course be tweaked by reducing foresight height if shooting low but most snubs are intended for most part to be used ''up close and personal'' :)

Many snubs seem to be nearest POA with 158's - lighter bullets often going lower. I think best is to try various ammo types and select what works best for the gun.

Maybe. I have a Bisley Vaquero that David Clements put a dovetailed front sight on. The sight was extra tall to allow for filing down for height and you can tap left or right for windage. Works well for my Vaquero, as it now shoots to POA at 25 yrds. Not sure if he offers that for the SP101 though, although there is plenty of steel there to work with. Ask him at clementscustomguns.com.

There is a factory "fix".

Ruger can change the front sight for a taller or shorter sight to correct shooting high or low.

For windage adjustment, they can turn the barrel slightly in or out to bring it on target.
Note that this adjustment is NOT one to try at home.
First, it takes special tooling to prevent damaging the frame, and second, there's more to it than just turning the barrel.
They also have to adjust barrel/cylinder gap, and other possible issues.

So, if you happen to get a pistol that shoots off target, the first step is to try different ammo to see if you can find one that targets better.

If not, send in the gun, along with a test target showing the grouping and the range and ammo you shot it with.

They can "tweak" the gun to bring the groups to the center.

Be very careful about allowing local gunsmiths do this.
Most of them DO NOT have the special tooling, and too many will just turn the barrel however they can, without even considering the other items that might need to be adjusted.

Windage alignment normally good?

Is the windage usually accurate on the .357 SP-101 3 1/16" barrel? Elevation issues would be easy to deal with.

My wife's 3" SP101 is a great shooter- right outta the box. No adjustment required. As accurate as anything else we own- right to POA. Don't fret over it- you'll be fine.

Not without spending a pile of money. If the windage is close, you can adjust the load to alter the elevation and a change of grips can change it as well. Mind you, a 3" revolver isn't a target gun. It's for close in shooting.
