Strange FTE and double fire today at the range

We were shooting my PPQ 9mm. We were using WWB 115 gr target ammo. I have over 100 rds throiugh the pistol now. My buddy was shooting it at about 120 rds out and then it double fired and FTE'd. He says he doesn't think he pulled the trigger twice. he is a gunsmith and a regular shooter. he said he thought it might be the disconnect didn't engage properly. But it also didn't eject the 2nd shell.
Other than that we fired about 60 rounds with no other hiccups. Very accurate gun, and my buddy loved it. Very accurate rapid fire.

What thinks the think tank here at THR? Could it have been the ammo combined with the disconnect failure?
 
if he did not have a solid grip when the second round went off the pistol could have moved too much to function the slide properly.
 
1/10" reset. I've seen this before, on video. The FTE part didn't happen, but an accidental twitch-fire did.

The guys at the Walther forums/PPQ sub forum are great resources.

Fwiw, I have 2,550 of all kinds of great and terrible ammo through mine with 0 malfunctions.
