I inherited a winchester 190 .22 from my grandfather,and had not taken it to the range until a few days ago when I took it along to the range when shooting my Saiga.Before leaving I cleaned the bore with a patch pulled back the bolt and cleaned the action.Everything looked ok but at the range I fired 1 round,inspected the casing and noticed the lip of the casing had a dent,so I put the 22 away and finished my session with the Saiga.Would the dent in the casing be from the shell being hung up during the cycling process,maybe from a need of a more in depth cleaning?
 
Probably wouldn't hurt a thing, also get yourself a set of new springs from Wolff Springs for it. It does sound as if the breech is all funky and in dire need of cleaning, new springs and things should clear up. Good luck.
 
It sounds like its getting dented on ejection. Try cycling an empty case through it slowly by hand and see what it is doing. If it ejects reliably, I wouldn't worry about a dent in a spent casing.

The Winchester 190 uses the firing pin as the ejector. When the bolt comes back, the firing pin is forced foward and the ejector part does its job. You could try taking the bolt out and cleaning the slot the firing pin rides in. be careful not to lose the spring and ball thats in there if you go as far as taking the bolt apart.

Other common problems with the 190 are a lose barrel and lose forearm. The few plastic parts also tend to shrink with age.
