Follow-up:
I had my guy put in a Wilson Combat Bullet Proof Extractor, and now it runs 9mm beautifully, several magazines, varied loads, fast, slow, it's all good. He had tried everything on the stock extractor, polishing, beveling, flexing, he could improve it a little bit, but never really made it consistent.
I will worry about the TCM later, maybe build a separate slide for it.
