Ar15 gas block leak

So i built a blackout sbr ar. Ballistic advantage barrel and gas tube, wojtek adjustable clamp on gas block. Never used wojtek, but they like him on Reddit.

Anyway, I'm shooting today for the first time and I'm getting carbon blown out of the gas block, through the handguard and in my hand. Never had this happen before. So I took it apart and this is what I see. (Attached pictures) how concerned should I be? Will it foul shut eventually? Why is the nitride gone off my barrel?

it is/was not closed all the way. that said, it is/was plenty tight. and held on with three clamping screws.

Gas block area of the barrel measured .750-.752, depending on how tight I clamped down on the calipers. I couldn't find my micrometer.
 
Are the clamp on type or the type where the screws tighten into the barrel (dimple) better? Or the same?
 
It’s possible that you just knocked the gas block out of position while shooting. That’s a problem that can happen sometimes when using a gas block that doesn’t use a set screw which tightens to a dimple on the barrel.
