Well, I'll be. I downloaded the PDF version of form 5310.16 and sent it to Kinko's to get it printed out. Took it home, filled it out and then began reading the instructions in detail... typical procedure for me.
On the second page under the section titled "INFORMATION FOR THE CHIEF LAW ENFORCEMENT OFFICER" reads:

So... What do questions #10 and #11 ask?
#10 is a checklist that you initial to verify that you're not breaking any local laws.
#11 is your signature.
It's not even possible to answer yes. They must mean questions #8 and #9 which is a series of yes/no questions that you see on the Yellow forms before the NICS check.
I just find it ironic, since we see stories of the ATF cracking down on people for small oversights in their bookeeping. Although I'm wondering if their website is dishing out this version mistakenly. I -hope- I've got an up to date copy of the form.... it was only downloaded a week ago.
The form is labeled: ATF F 7CR (5310.16) (6-97)
I take that to mean the form was stamped for approval in July 1997. Has this mistaken ridden for 6 whole years?

This form provides notification of a person's intent to apply for a Federal firearms License. It requires no action on your part. However, should you have information that may disqualify the person from obtaining a Federal firearms license, please contact the Firearms and Explosives licesning Center at (404) 679-5040. A "Yes" answer to questions #10 and #11 could disqualify a person for a license. Also, ATF may not issue a license if the activity would be in violation of State or local law.
