i can't speak for others but my biggest "what was i thinking!" moments often have a strong testosterone component. sprinkled liberally with ego. i have come to recognize these warning signs and try to leave when i sense them. quitting drinking also reduced the frequency and intensity of incidents.
 
I just finished Amanda Ripley's The Unthinkable it is an exceptional and engaging read; I recommend it for anyone. I can see how just reading it could enable a person to make a real difference in an emergency situation. And I know that I will do little things like taking the stairs down once when I get a hotel room.
