I witnessed a competitor at a USPSA match last night shoot the lead out of a bullet leaving the copper jacket lodged in the barrel, twice. He was shooting very hot 9mm's (loaded long to make major). I have shot tens of thousands of hand loaded rounds and never seen anything like it.

Any ideas on the cause....

don't know about now,but several years ago had a friend that bought about 5000 Zero JHP bullets, and the things rattled.you could take a bullet ,shake it and it would rattle.the lead wasn't tight in the jacket material. jwr

Faliure to launch

"Houston, we have a problem..."

That happens with high velocity jacketed rounds that have uncovered lead at the tip and the base at the same time. The lead shoots out leaving the jacket(which has a higher friction coeficient), behind. That's why you should not try to make a FMJ into a JHP by simply filing off the tip. All factory JHPs cover the base of the bullet.
