Check that the extractor and breechface are clean. Load a dummy round or an empty cartridge into the chamber, then pull the slide back partway. The cartridge should be held tightly to the breechface all the way until the ejector kicks it out. If it drops out, or if you can shake it loose, there's a problem. It could be a bent or poorly fitting extractor.
Even if it appears to be held securely, the tension on the extractor could be too low due to a weak extractor spring.
You can also give the ejector (not extractor) a little polish on the point of contact to ensure the brass rebounds smoothly off it without upsetting the grip.
On one of my autos, I had the same FTE's. It's a really crappy malfunction to have, since tap-rack doesn't work. I also noticed that hand cycled rounds dribbled out the side, hitting my hand as they rolled out of the ejection port. After finding and correcting the extractor issues (using a Dremel to reshape the extractor and a shim for the extractor spring!), these rounds now leave dents on the wall clear across the room. And the FTE's went away.
