G19 slide lock/FTE issues since Acro

I have a gen 2 G19. Gen 3s are hard to get in MA. Ever since putting an acro on my slide (no adapter plate, acro-specific milling) months back I've had pervasive FTE issues, often in the form of a "double feed" against the partially ejected empty casing. I've also at least once had the slide lock back when there was still ammo in the mag. More often than not I can't make it through a 5 round string of fire without a malfunction.
I thought my thumb meat was contacting the slide release with a difference in recoil or something, but 5 different slide releases later (including kagwerks elevated release), no improvement.
I tried my slide assembly on a friend's gen 3 frame and had no issues. Took their weapon light off in case in was acting as a counterbalance. Still no issues. And they have an extended slide release that I previously had on mine. I'm just baffled. Any ideas?

I'm wondering if the subject G19Gen2 is a 2-pin type of Glock. If so, there's a special part#399 slide stop lever with a spike on top.
I've never worked with an older Gen2 and don't quite understand what that spike does, however.

In regard to your FTE issue, the screw securing your Acro to the slide may be touching the extractor rod (depressor plunger). Remove the slide backplate then remove the rod. If the rod doesn't come out then loosen the Acro screw right above it. Once you have the rod out, tighten the screw and look to see if the tip is protruding into the rod channel. If it is then remove the screw and install a couple of nuts onto it until two or three threads are protruding out of them then lock the nuts together. Put in a vise and file off the 2-3 threads, remove the nuts and reinstall the screw. Repeat until the tip of the screw no longer protrudes into the rod channel.
