Has anyone ever actually witnessed a decocker fail and discharge the gun?

A friend of mine put a round through his window with a CZ-75 marked "POLICE" that I had acquired used, never had an issue with, and sold to him.

I was there and it was indeed a malfunction. He definitely didnt pull the trigger as I saw it and as he saw it / felt it. We were shocked to say the least.
 
Some CZ75s have no FPB. I don't think any decocker models do. It's a pretty beefy FPB. I can't imagine the firing pin making it through the block without a level of force that would mangle the entire firearm.

I do regularly manually decock on the manual safety versions though. It is theoretically safe. The key is to have some flesh between the hammer and pin when you start to lower it, and let off the trigger immediately. As with the decocker, dropping a hammer with the trigger/FPB in the disengaged position won't allow a round to fire. It's not something I'd trust any random person to do, though.
 
Never happened to me, but a guy had a Walther PP of some type at the local outdoor range about 10 years ago, and off it went. Scared him bad enough that he loaded up and went home. Scared me and a couple of friends, too, but we kept on shooting.
